Case Management

The Case Management department is part of a collaborative multidisciplinary hospital-wide process that is available to assist patients and their families develop a feasible post-hospital plan of care.  The Case Management staff will provide psychosocial assessment and intervention when appropriate.  They will assist with social issues and will act as a referral source, information source and liaison for patients, families, physicians and other members of the multidisciplinary team.

The Case Management department will also assist patients when trying to make difficult end of life decisions by providing information and resources for Living Wills, and Power of Attorney for Healthcare.
